Duty to Report

Worker

When a worker is injured on the job and requires medical attention off the job site and/or needs time off work, beyond the day of their injury, it is their responsibility to fill out, sign and submit a Worker’s Report of Injury/Illness to the Yukon Workers’ Compensation Health and Safety Board.

The worker will also provide a copy of this form to the employer.

If the worker’s injury or illness requires medical attention, inform the doctor you have a work-related injury/illness.  The doctor will fill out and submit to YWCHSB, the Doctor’s First Report.

Employer

Make sure your worker(s) know they are covered by workers’ compensation and they know what to do if they are hurt at work.

If a worker is hurt, the employer must inform Yukon Workers' Compensation Health and Safety Board immediately if the injury or incident is serious (if an ambulance was called, it's serious enough to qualify).

For workplace injuries where no medical attention is required beyond first aid in the workplace, or time off beyond the day of injury, the employer must keep a written record of the incident (but does not have to send a written report to YWCHSB).

For any workplace injury that requires medical attention beyond first aid in the workplace, or time off beyond the day of injury, the employer must file a report with YWCHSB within 3 days.
