Checklist for Workers

  • Did you get medical attention? If it is required, obtain it as soon as possible. Advise your doctor if your condition is work-related. Your doctor will send a report to the Yukon Workers' Compensation, Health and Safety Board.
  • Did you notify your employer? Do this as soon as possible. Give them full details of the injury or illness, and the names of any witnesses.
  • Did you fill out a "Worker's Report of Injury/Illness"? Do this only if you have received medical attention off the job site and/or needed time off work, other than the day of the injury. Send the original to the YWCHSB, give your employer a copy, and keep a copy for your records. You have one year to submit your report to the YWCHSB.

Keep in mind that you are expected to participate in your treatment plan with your physician, adjudicator and employer to speed your return to work.
