Update your business contact information by completing the NEW online form.

Keeping employer contact information up to date helps WSCB provide timely communication and better support for compliance requirements. This includes business addresses, phone numbers, email contacts, and key personnel.

Your contact information is used for important correspondence, including claims requests for wage information, decision letters, invoices, Employer Payroll Returns, and other communications. Keeping your information current helps ensure you receive documents and updates in a timely manner.

For third party representation please complete the Authorization of a Representative form. This will authorize the WSCB to discuss your account with a person outside of your organization.

If incorporated, use the legal name as listed on your Articles of Incorporation. If not incorporated, the legal name should be the name of the owner(s).
The trade name is the name most recognized by the public and customers.
This number can be found in the email we sent you to complete your annual report or on any of your invoices or statements. Example: 12345-54321
