A staff person will review your application and, if required, get in touch with you for clarification or more information.
Once we have all information required to process the request and the applicable fees (if any), you can expect to receive the disclosure package within 10 business days (not including statutory holidays).

NOTE: If disclosure packages are not picked up within 30 days, they will be destroyed. If disclosure is still required, a second Request for Disclosure of Claim Information must be submitted (fee applies).
